Becoming a Student-Athlete

Shelbi Dawkins MS, LAT, ATC is our athletic trainer. She earned her bachelor of science in pre-athletic training and master of athletic training from the University of Idaho. Athletic training services include the following:
- Athletic training coverage at home competition and practices
- Injury evaluations/screening performed on campus
- Preventative care
- Emergency care
- Priority access to sports medicine trained physicians and orthopedics
- Priority access to physical therapy, sports performance, and recovery services at Youngs Physical Therapy & Sports Performance
THE DEADLINE TO COMPLETE ALL REQUIREMENTS TO PARTICIPATE IN SPORTS FOR THE FALL 2025 SEASON IS MONDAY, AUGUST 4TH
Healthy Roster is the electronic medical records (EMR) system that JPII utilizes to securely collect forms related to athlete eligibility and documentation of athletic injuries/illnesses. It is a REQUIREMENT of student-athletes to successfully set up an account and complete the necessary forms.
Families can use the website version of Healthy Roster or the Apple or Android mobile app to upload documents and complete forms.
In order to create an account, you should receive an email from Healthy Roster including a code and links to the website/App Store to download. If you have not received one, reach out to Ms. Shelbi at sdawkins@jp2chs.com. Once you have downloaded the app or navigated to the website, select Sign Up on the log-in screen, and then input the code received in your email. Next, you will input your email and other necessary information to create your account (phone number, password, etc.). Once that is done, you officially have access to the account and are welcome to explore the features you now have access to.
Once you have downloaded the Healthy Roster app and claimed your student(s), the forms below need to be uploaded/completed in Healthy Roster prior to participation in workouts, practices, or games (this includes offseason).
- Concussion Vital Signs Baseline Test conducted by the athletic trainer (see below)
- a sports physical form dated within the last 365 days (see below)
- Concussion Baseline Assessment Permission Form reviewed and signed by student-athlete and parent/guardian
- NCISAA Concussion Information Form completed and signed by student-athlete and parent/guardian
- NCISAA Consent to Participate and Release Form reviewed and signed by student-athlete and parent/guardian
- John Paul II Release of Participation and Transportation Policy Waiver completed and signed by student-athlete and parent/guardian
All forms listed on the Healthy Roster must be completed prior to participation in workouts, practices, or games (this includes offseason) in order for an athlete to be listed as “eligible.” Please DO NOT turn in any HARD copies of signed documents to your coaches or Ms. Shelbi. All forms should be completed/uploaded electronically via Healthy Roster.
Pre-Participation Physical
The North Carolina Independent School Athletic Association (NCISAA) requires that Pre-Participation Physical Examinations be completed every 13 months.
- History Form – Page 1:
- Completed by the student-athlete and parent/legal guardian
- History Form – Page 2:
- Completed/signed by student-athlete and parent/legal guardian
- Physical Examination Form – Page 3:
- Performed by and signed/dated by healthcare professional (MD, DO, NP, or PA)
- Medical Eligibility Form – Page 4:
- Completed by and signed/dated by healthcare professional (MD, DO, NP, or PA)
- Appropriate clearance box must be checked
- Completed by and signed/dated by healthcare professional (MD, DO, NP, or PA)
- Hardcopy uploaded to Healthy Roster account via PDF/picture file
- Pages 1-4 must be uploaded to be valid
Concussion Vital Signs Baseline Testing
Concussion Vital Signs (CVS) is an annual requirement for all student-athletes at John Paul II Catholic High School.
We use the software “Sway” in order to complete testing. Students must arrive with the app “Sway Medical” downloaded on their phone (Apple or Android) in order to complete the test. In order to schedule a baseline test, please contact Athletic Trainer Shelbi Dawkins at sdawkins@jp2chs.com. There is no cost for this test.
Coaches’ Contact Information
Click here to find a list of all sports currently offered, head coaches’ names, and contact information. Students are encouraged to go ahead and contact coaches to let them know they are interested in playing (regardless of when the sport starts).
